Purpose:

To evaluate the visual outcome and wavefront aberrometry after implantation of accommodative posterior chamber intraocular lens(WIOL-CF).

Methods:

In this retrospective case series study, we analyzed 40 eyes of 40 patients (aged 40-77 years) after cataract surgery with implantation of accommodative WIOL-CF.preoperative and postoperative distance and near visual acuity measurements were performed with standard snellen chart at 20 foot and Birkhauser respectively,for all patients. For all patient were done topography preoperatively,inaddition all of them were tested with itracy device(Tomey.Inc.) for evaluation of Modulation Transfer Function(MTF), Point Spread Function(PSF), total order aberrations, higher order aberrations ( total,coma,spherical- aberration trefoil and secondary astigmatism) at far and near and pseudoaccommodation measurement postoperatively in the last follow up

Results:

The mean LogMAR CDVA improved from 0.20±0.14( 0.00-o.54) before surgery to 0.10±0.09( .00-0.30) at the last follow up that is statistically significant. No loss of line of CDVA was happened .The result of UNVA is 60% J1, 70%J2, 85%J3, 90J4, 95% J5 and 100% J6. The mean accommodation, range of accommodation volume and average of peak accommodation were - 2.52 ±1.56 diopter, 1.50 to 5.25 diopter and – 3.25 ± 1.25 diopter respectively. The mean MTF at 5 ,10 ,15 ,20 ,25 ,30 cycle per degree at far included ; 0.200±0.10, 0.084±0.04, 0.065±0.06, 0.040±0.02 , 0.030±0.01 and 0.024±0.01. These are at near included ; 0.207±0.10, 0.089±0.04, 0.055±0.02, 0.042±0.02, 0.033±0.01, 0.027±0.01. The mean MTF at near in all spatial frequency except at 15 cpd was increased relative to far but these changes are not statisticaly significant. The mean RMS wave front error of total order aberration(TOA) at far and near were 2.75±1.66 µm and 2.14±1.53µm respectively, there was a reduction at near that is statistically significant. The mean RMS value of high order aberration(HOAs); total, coma spherical aberration, trefoil and secondary astigmatism were decreased at near relative to far, so that is statistically significant

Conclusion:

our results demonstrated an acceptable accommodative capacity of the WIOL-CF with good uncorrected near and ditstance visual outcome.
