Title: Comparison of patients with low and high preoperative corrected distance visual acuity undergoing trans-epithelial photorefractive keratectomy in terms of surgical outcomes

Purpose:

To compare optical objective indices in patients with preoperative corrected distance visual acuity above 10/10 and below 10/10

Methods:

Seventy-nine eyes (51 patients) with different refractive errors were recruited in a prospective study from July 2013 to September 2014. All of the cases underwent Refined Trans-PRK by the same expert surgeon (S.A.M.) using SCHWIND AMARIS 500 laser. Visual acuity, refraction, CS and high order aberrations (HOA) were registered in pre-op and post-op visits. Patients were divided into two groups: preoperative CDVA ≤10/10 and >10/10.

Results:

Fourty-eight eyes belonged to low CDVA group and 31 eyes belonged to high CDVA group. Mean age of low CDVA group was significantly higher than high CDVA group (29.26±5.00 vs 26.45±3.74, P=0.006). F/M ratio was comparable in two groups and 2.7 in total. Mean follow up was 226 days (154-371 days). Post-op CDVA was comparable in two groups (1.22±0.23 vs 1.25±0.44, P = 0.7). Post-op UDVA was higher in high CDVA group (1.31±0.37 vs 1.15±0.23, P =0.001). Pre-op mean manifest SE was more myopic in low CDVA group (-4.41±2.02 vs -3.28±1.41, P = 0.001) while post-op SE were comparable (-0.02±0.16 vs 0, P =0.47). Mean post-op photopic and mesopic contrast sensitivities were better in high CDVA group (respectively 0.95±0.58 vs 0.76±0.16, P=0.04 and 1.00±0.58 vs 0.80±0.1, P=0.01); however mean improvement percentages of both photopic and mesopic contrast sensitivities were higher in low CDVA group (respectively 11.98±3.97% vs 5±4.21%, P=0.04 and 20±5.57% vs 3.63±2.02%, P = 0.001). Pre-op mean Defocus and Pentafoil aberrations were higher in low CDVA group. After the operation they became comparable. Post-op mean coma aberration was higher in high CDVA group. Other pre-op and post-op aberrations were comparable.

Conclusion:

In patients with low pre-op CDVA, one-step Refined TransPRK could improve CDVA and refraction so that these indices were comparable with high CDVA group after the operation. Additionally, this operation could notably improve photopic and mesopic contrast sensitivities as well as higher order aberrations in this group.
