Purpose:

To evaluate the outcome of second Ahmed Glaucoma Valve (AGV) surgery after first AGV failure

Methods:

37 eyes of 34 patients with history of Ahmed Glaucoma Valve implantation after previous failed AGV surgery for uncontrolled glaucoma were enrolled in this study. Main outcome of this study was success rate according to two criterions with respect to different target intra ocular pressure. Then Data analysis was performed using SPSS software version 20.

Results:

Mean age of patients was 28.97±18.69 (range4 – 65). Mean duration of follow up was 21.43±21.71 months (range 6 – 96 months). Mean preoperative IOP was 26.94±7.03mmHg (range 13 - 50 mmHg) with 2.8±0.9 number of IOP lowering medications (range 0 - 4) and mean post operative IOP was 13.81±5.3 mmHg (range 5 - 22) at month 6 (p=0.00)and 13.28±3.59 ( range 5-28 ) at last visit(p= 0.00) . Mean IOP lowering medications at month 6 was 1.49±1.07 (range 0 - 4) (p=0.000) and Mean IOP lowering medications at last visit was 1.42±1.13 (range 0 – 3) (p=0.000). Overall success at last follow up was 75.7% according to criterion A and 54.1% according to criterion B.

Conclusion:

Repeated AGV insertion seems to be a good modality with acceptable success rate in cases with failed previous AGV surgery.
